Please complete new forms for your scout(s) every year. Be sure to include a copy of the front/back of medical insurance card(s) and vaccination records (including COVID vaccine). Part C, which can only be completed by a physician, is only necessary for scouts and leaders attending Camp Conestoga this Summer.
